This three-day event was packed with education, networking opportunities, and even a special concert at the iconic Grand Ole Opry. The vibe? Electric! Participants shared ideas, connected with peers, and immersed themselves in the latest trends and best practices in case management. Membership Milestone and Awards

In a sign of its growing influence, the ACMA has seen its membership soar, surpassing 20,000 members nationwide. To honor dedication and excellence, the ACMA awarded the prestigious Case Management Hero Award to three outstanding individuals in 2024. Even more impressively, this tool has been utilized by over 70% of U.S. hospitals.

Celebrating a Unique Week and New Media

National Case Management Week was also celebrated in tandem with the Commission for Case Manager Certification, under the engaging theme, “Case Management: Encouraging, Engaging & Empowering.” This week not only recognizes the hard work of professionals in the field, but also serves as a reminder of their vital role in the healthcare continuum.

Adding to the excitement, ACMA launched a new podcast series titled “Care Transitions Today: Case Conversations.” This engaging audio series takes listeners through real-world medical cases and discusses practical best practices for engaged case management and care transitions.

In line with its commitment to excellence, ACMA revised its original Transitions of Care (TOC) Standards in collaboration with Pfizer. The newly unveiled TOC 2.0 standards now place a stronger emphasis on addressing Social Determinants of Health (SDOH)—an essential aspect of quality patient care today.
