In the heart of Michigan, something special is brewing for rural communities! The Michigan Center for Rural Health (MCRH) has kicked off a new phase of a vital initiative that aims to enhance palliative care in areas where access to healthcare can be a challenge. This comes on the heels of recent studies that reveal a pressing demand for these services in rural settings, including our beautiful state of Michigan.
MCRH is teaming up with Stratis Health, a nonprofit consulting organization known for its expertise in supporting rural healthcare endeavors. Together, they’re launching an initiative known as “Cultivating Comfort: Enhancing Palliative Care in Rural Michigan,” which is being generously funded by the Michigan Health Endowment Fund.
The first phase of this project was all about understanding the landscape. In 2024, MCRH conducted a statewide assessment and gathered input from a diverse advisory council to shape the next steps. Now, Phase 2 of the initiative has officially started as of January 1, 2025, and it’s set to run for two years. So, what can we expect from this jam-packed phase?
The goals for the upcoming two-year stretch are pretty ambitious! The project aims to:
Communities interested in benefiting from this initiative are currently invited to submit applications. The deadline is February 16, 2025, so there’s no time to waste! When choosing the five rural communities that will participate, selection criteria will include factors such as the level of rurality, community engagement, and any existing palliative care efforts.
One of the most exciting aspects of this project is its community-driven and flexible nature. The aim is to create a model of palliative care that’s not only effective but also easily adaptable to meet local needs. To make this happen, collaboration with local organizations is crucial, ensuring that palliative care is integrated with existing services provided by local healthcare providers.
Furthermore, the initiative will adopt a multidisciplinary approach, incorporating professionals from various fields such as physicians, nurses, social workers, chaplains, and family caregivers. This holistic view is key to providing comprehensive care that addresses the physical, emotional, and spiritual needs of patients.
As the project unfolds, training will be a big part of the process. Staff at MCRH will receive training on Stratis Health’s palliative care framework, ensuring they’re equipped to facilitate effective implementation in the chosen communities. Once selected, a gap analysis will be performed in each community to pinpoint local needs and available resources, informing the design and delivery of services.
Cross-collaboration among the participating communities is encouraged, with a focus on sharing best practices and valuable lessons learned throughout the project. An important part of the initiative also involves educating healthcare providers on palliative care, which includes offering webinars for continuing education credits.
The ultimate goal? By the end of this initiative, MCRH aims for participating communities to establish sustainable palliative care plans that can thrive long into the future. Special attention will also be given to improving advanced care planning, symptom management, communication, and coordination of care for those grappling with chronic illnesses.
What’s more, there’s a significant interest in partnering with Native American communities as part of this rural outreach effort. It’s all about recognizing and understanding the unique challenges faced by rural populations in achieving quality palliative care.
